สิ่งที่ส่งมาด้วย 1) ใบขอทำบัตรประจำตัว TRANSFER-MAN ประจำปี 2557

ด้วยสมาคมไทยธุรกิจการท่องเที่ยว (ATTA) กำหนดให้จัดทำบัตรประจำตัว TRANSFER-MAN ประจำปี 2557 สำหรับตัวแทนของบริษัทสมาชิกที่ไม่ได้เป็นมัคคุเทศก์ เช่น แอร์พอร์ตเรฟ เจ้าหน้าที่บริษัท หรือพนักงานขับรถ สามารถยื่นเรื่องขอทำบัตรประจำตัว TRANSFER-MAN ประจำปี 2557 เพื่อใช้ในการแลกบัตรรักษาความปลอดภัย เข้าพื้นที่ควบคุมบริเวณชั้น 2 ห้องโถงผู้โดยสารขาเข้า (ประตู 3 – 10) ท่าอากาศยานสุวรรณภูมิ ได้ตั้งแต่วันที่ 2 ธันวาคม 2556

เพื่อเป็นการอำนวยความสะดวกและให้มีความรวดเร็วยิ่งขึ้นในปีนี้ สมาคมฯ ใคร่ขอความร่วมมือจากผู้ที่ประสงค์จะทำบัตร TRANSFER-MAN ประจำปี 2557 โปรดเตรียมเอกสารให้ครบถ้วนและนำมายื่นด้วยตนเอง พร้อมกับถ่ายรูปติดบัตรได้ที่สมาคมไทยธุรกิจการท่องเที่ยว (ATTA) สำนักงานใหญ่ เพื่อที่จะรับบัตรได้ทันที

หลักฐานการทำบัตรประจำตัว TRANSFER-MAN ประจำปี 2557

1. ใบขอทำบัตรประจำตัว TRANSFER-MAN ประจำปี 2557

2. รูปถ่ายสี 1 นิ้ว จำนวน 1 รูป
3. สำเนาบัตรประจำตัวประชาชน จำนวน 3 ชุด (เซ็นรับรองสำเนาถูกต้อง)
4. สำเนาทะเบียนบ้าน จำนวน 3 ชุด (เซ็นรับรองสำเนาถูกต้อง)
5. จดหมายรับรองการเป็นพนักงานของบริษัท
6. สำเนาหนังสือรับรองบริษัท
กรณี TRANSFER-MAN ที่สังกัดหลายบริษัท

ผู้สมัครจะต้องส่งใบสมัครให้สมาคมฯ ตามจำนวนที่สังกัด พร้อมจดหมายรับรองการเป็นพนักงานของบริษัท และสำเนาหนังสือรับรองบริษัทของแต่ละบริษัท มาพร้อมกัน ซึ่งสมาคมฯ กำหนดให้ TRANSFER-MAN 1 คน สังกัดได้เพียง 3 บริษัท เท่านั้น

ท่านสมาชิกสามารถติดต่อขอรับแบบฟอร์มการทำบัตรประจำตัว TRANSFER-MAN ประจำปี 2557 ได้ที่

การยื่นเรื่องทำบัตรประจำตัว TRANSFER-MAN ประจำปี 2557

ผู้ประสงค์ที่จะบัตรประจำตัว TRANSFER-MAN ประจำปี 2557 จะต้องเตรียมเอกสารให้ครบถ้วน และนำมายื่นด้วยตนเอง พร้อมกับถ่ายรูปติดบัตรได้ที่สมาคมไทยธุรกิจการท่องเที่ยว (ATTA) สำนักงานใหญ่ 33/42-43 อาคารวอลล์สตรีททาวเวอร์ ออฟฟิศคอนโดมิเนียม ชั้น 10 ถนนสุรวงศ์ แขวงสุริยวงศ์ เขตบางรัก กรุงเทพฯ 10500 เพื่อที่จะรับบัตรได้ทันที

คุณสมบัติของผู้สมัครทำบัตรประจำตัว TRANSFER-MAN

ผู้สมัครทำบัตรประจำตัว TRANSFER-MAN จะต้องมีเชื้อชาติไทย สัญชาติไทย เท่านั้น
อายุการใช้งานของบัตรประจำตัว TRANSFER-MAN

บัตร TRANSFER-MAN มีอายุการใช้งาน 1 ปี (มกราคม – ธันวาคม 2557)

ค่าใช้จ่ายในการทำบัตร 300 บาท/คน/ปี (รวมภาษีมูลค่าเพิ่มแล้ว)

Subject: Requesting for the Transfer-Man Card 2014

Dear ATTA Members,

The Association of Thai Travel Agents (ATTA) would like to inform members about requesting for the Transfer-Man Card for the year 2014. This is being conducted for members’ representatives who are not tour guides; such as, airport reps, staff or drivers and can request for the Transfer-Man Card 2014. This is to exchange the card for security purposes when entering the restricted zone in the Arrivals Hall on the 2nd Floor (Gates 3-10) at Suvarnabhumi Airport that would be effective as from 2 December, 2013, onward.

In order to provide facilitation and even more rapidity this year, ATTA would like to request for the cooperation from prospective Transfer-Man 2014 cardholders to prepare all the necessary documents and submit them in person together with a photograph at the ATTA Head Office in order to receive the card immediately.

Requirements for the Transfer-man Card 2014

1. Application form for the Transfer-man card 2014

2. 1 colour photo 1 inch in size.

3. 3 copies of the Thai Citizen I.D. Card (clearly signed as being ‘True Copies’).

4. 3 copies of the house registration (clearly signed as being ‘True Copies’).

5. Letter of confirmation from the company stating that the person is employed by that company.

6. Copy of a document from the company signed by the Board of Directors/Proprietors

In the case, the Transfer-man is under the service of multiple companies

The applicant must send an application form for each company to ATTA together with a letter of confirmation from the company stating that the person is employed by that company as well as a copy of a document from each company signed by the Board of Directors/Proprietors. ATTA has set the stipulation that 1 Transfer-Man can be employed by a total of 3 companies.

Submission of Documents
Cardholders must prepare ALL documents submit them in person with a photograph at the ATTA Head Office, 10th Floor, Wall Street Tower, Surawong Road, Bangrak, Bangkok 10500, so to receive the card immediately.
Qualifications

Must be a Thai national.

Validity

The card is valid for 1 year (January-December 2014).

300 Baht / person / year (includes VAT).
